2) Next, if you also qualify to join the Ordinariate, please print out, fill out and sign, and turn in ONE ORDINARIATE FORM PER INDIVIDUAL FAMILY MEMBER.

To turn in: place the completed form in the dropbox next to the main doors. Do NOT mail this in directly to Houston; we will do this for you after completing your registration in the parish first.
